To 1)
As you can see in the GTA04 handbook, the modem is an Option GTM601W,
which is a Quadband-GSM and Quadband-UMTS device.
This means, GSM 850/900/1800/1900 and UMTS 850/900/1900/2100 [1]

To 2) It's a TI DM3730 with PowerVR SGX 530
